The application form has been revised and two separate versions are now available for people who prefer to complete an online application and for those who like to handwrite their applications. The application form for an INDIVIDUAL is available in either Word or PDF format. You may download the Word version of the application to your computer, then fill it in, save it, and send it to the Council's office through the US postal service or email it as an attachment to Info@icdd.idaho.gov. You also have the option to print it out as a PDF, then complete it and mail the hard copy to the Council office.
Follow the same procedure for an ORGANIZATION'S conference request. · Organization Conference Request in Word format · Organization Conference Request in PDF Disability Mentoring Day is an annual effort of the American Association of People with Disabilities that brings students and other young adults together with employers for informational sessions about career opportunities and one-on-one mentoring with volunteers at public and private places of employment. The form to apply for Council funding for this project is available in several different formats for the application. ** **The applications for 2008 are closed. Please check back for 2009 applications. Disability Mentoring Day for 2009 has not been scheduled. Training Sponsored by the Council: The Council sponsors miscellaneous trainings which may require an application be completed and sent to the Council for approval. Those applications will be listed when they become available. Partners in Policymaking Program (see Partners Web page) is one of the well-known training programs sponsored by the Council on Developmental Disabilities.

Youth Leadership Forum is a leadership, citizenship and career development program for high school juniors and seniors with disabilities. Selected youth serve as delegates from their communities at a statewide or regional conference. Student delegates gain information and develop personal leadership plans. The program is intended to assist youth to realize their abilities, rights and responsibilities to pursue meaningful employment and become leaders in their community. The 2009 Youth Leadership Forum (YLF) application is now available in Word and PDF.
